Status of hyperhomocysteinemia in China: results from the China Stroke High-risk Population Screening

Frontiers of Medicine 2021, Volume 15, Issue 6,   Pages 903-912 doi: 10.1007/s11684-021-0871-4

Abstract: A nationwide survey was conducted from October 2018 to September 2019 to assess the prevalence of hyperhomocysteinemia (Hhcy) and its influencing factors in China. A standardized questionnaire was used to collect information. Hhcy was defined as the level of serum homocysteine (HCY) 15.0 µmol/L. The H-type hypertension (HHYP) was defined as hypertension with an elevated serum HCY (15.0 µmol/L). Finally, 110 551 residents 40 years of age from 31 provinces in the mainland of China were included. Overall, the median serum HCY level was 10.9 µmol/L (interquartile range 7.9–15.1). A total of 28 633 participants (25.9%) were defined as Hhcy. The Hhcy prevalence ranged from 7.9% in Shanghai to 56.8% in Tianjin. The data showed that serum HCY levels were associated with age, male gender, cigarette smoking, hypertension, diabetes, ethnicity, endurance in exercise (inverse), and fruit and vegetable intake (inverse). In addition, 15 486 participants were defined as HHYP, and the rate was 14.0%. HHYP was an independent predictor of stroke with an adjusted odds ratio of 1.752 (95% CI 1.338–2.105). The geographical distribution pattern of the Hhcy epidemic reflects dynamic differences, and national strategies should be carried out to further improve the care of patients with Hhcy across China.

Preliminary results of gentamycin combined with sodium bicarbonate for prevention of irinotecan-induced

Qi MEI MM, Zhe CAO MM, Hua XIONG MD, Yuan CHEN MD,

Frontiers of Medicine 2009, Volume 3, Issue 4,   Pages 470-474 doi: 10.1007/s11684-009-0077-7

Abstract: The aim of this paper was to observe the clinical effect of gentamycin combined with sodium bicarbonate for the prevention of irinotecan-induced diarrhea. A total of 98 patients with stage IV cancers were recruited and divided into a prevention group (52 patients) and a control group (46 patients). All patients received the chemotherapy including irinotecan. The prevention group received gentamycin and sodium bicarbonate before the use of irinotecan for 4 days; the control group did not receive any prevention. The use of gentamycin and sodium bicarbonate resulted in significantly higher stool pH ( < 0.001), while the incidence of diarrhea by irinotecan was reduced (prevention group 13.70% control group 34.83%; < 0.001). Gentamycin combined with sodium bicarbonate appears to be useful in preventing the diarrhea induced by irinotecan and reducing the dosage of loperamide and fluid replacement.

improved survival and increased response to immune checkpoint blockade in metastatic uveal melanoma: results

Frontiers of Medicine   Pages 878-888 doi: 10.1007/s11684-023-0993-y

Abstract: Metastases of uveal melanoma (UM) spread predominantly to the liver. Due to low response rates to systemic therapies, liver-directed therapies (LDT) are commonly used for tumor control. The impact of LDT on the response to systemic treatment is unknown. A total of 182 patients with metastatic UM treated with immune checkpoint blockade (ICB) were included in this analysis. Patients were recruited from prospective skin cancer centers and the German national skin cancer registry (ADOReg) of the German Dermatologic Cooperative Oncology Group (DeCOG). Two cohorts were compared: patients with LDT (cohort A, n = 78) versus those without LDT (cohort B, n = 104). Data were analyzed for response to treatment, progression-free survival (PFS), and overall survival (OS). The median OS was significantly longer in cohort A than in cohort B (20.1 vs. 13.8 months; P = 0.0016) and a trend towards improved PFS was observed for cohort A (3.0 vs. 2.5 months; P = 0.054). The objective response rate to any ICB (16.7% vs. 3.8%, P = 0.0073) and combined ICB (14.1% vs. 4.5%, P = 0.017) was more favorable in cohort A. Our data suggest that the combination of LDT with ICB may be associated with a survival benefit and higher treatment response to ICB in patients with metastatic UM.

Effectiveness of lifestyle intervention for hypertension in Shanghai communities: Results from the Shanghai

Xin-Jian LI MD, Min-Na CHENG MPH, Yu-Heng WANG MD, Sun MIAO MPH, Zong-Qi ZHANG PhD, Yi-Sheng CHEN MD, Wei LU PhD,

Frontiers of Medicine 2010, Volume 4, Issue 1,   Pages 67-70 doi: 10.1007/s11684-010-0023-8

Abstract: A community-based multi-center randomized controlled trial was conducted to evaluate the effectiveness of blood pressure control for hypertension patients in communities in urban Shanghai by integrated intervention. At present, patients (=1395) from four communities have completed follow-up for one year, including the intervention group (=921) and usual care group (=474). The intervention programs included disease management by a care manager. Blood pressure of each patient was measured regularly. Compared with the control group, the net change of mean systolic blood pressure (SBP) was −6.75 (95% CI: −7.79 to −5.71, <0.001) mmHg, mean diastolic blood pressure (DBP) was −4.29 (95% CI: −5.08 to −3.49, <0.001) mmHg, and mean pulse pressure (PP) was −2.46 (95% CI: −3.50 to −1.43, <0.001) mmHg in the intervention group. The net change extent was larger in patients with regular pharmacological treatment than in those with irregular pharmacological treatment or non-pharmacological treatment. The measures of integrated intervention for hypertension patients in communities can lower significantly not only SBP and DBP, but also PP. It is suggested that measures of integrated intervention can decrease the risk of cardiovascular diseases in hypertension patients.

Long-term results of suppressing thyroid-stimulating hormone during radiotherapy to prevent primary hypothyroidism

Maura Massimino, Marta Podda, Lorenza Gandola, Emanuele Pignoli, Ettore Seregni, Carlo Morosi, Filippo Spreafico, Andrea Ferrari, Emilia Pecori, Monica Terenziani

Frontiers of Medicine 2021, Volume 15, Issue 1,   Pages 101-107 doi: 10.1007/s11684-020-0752-2

Abstract: Primary hypothyroidism commonly occurs after radiotherapy (RT), and coincides with increased circulating thyroid-stimulating hormone (TSH) levels. We tested therefore the protective effect of suppressing TSH with L-thyroxine during RT for medulloblastoma/PNET and Hodgkin lymphoma (HL) in a prospective cohort study. From 1998 to 2001, a total of 37 euthyroid children with medulloblastoma/PNET plus 14 with HL, scheduled for craniospinal irradiation and mediastinum/neck radiotherapy, respectively, underwent thyroid ultrasound and free triiodothyronine (FT3), free thyroxine (FT4), and TSH evaluation at the beginning and end of craniospinal iiradiation. From 14 days before and up to the end of radiotherapy, patients were administered L-thyroxine checking every 3 days TSH to ensure a value<0.3 μIU/mL. During follow-up, blood tests and ultrasound were repeated; primary hypothyroidism was considered an increased TSH level greater than normal range. Twenty-two/37 patients with medulloblastoma/PNET and all the 14 patients with HL were alive after a median 231 months from radiotherapy with 7/22 and 8/14 having correctly reached TSH levels ˂ 0.3 μIU/mL and well matched for other variables. Twenty years on, hypothyroidism-free survival rates differed significantly, being 60%±15% and 15.6%±8.2% in TSH-suppressed vs. not-TSH suppressed patients, respectively ( =0.001). These findings suggest that hypothyroidism could be durably prevented in two populations at risk of late RT sequelae, but it should be confirmed in a larger cohort.
